Now for the boring stuff.

Month to month contracts will be automatically withdrawn from your account. A current credit card must be kept on file. If your payment does not go through there will be a grace period of 7 days and your card will be charged again. If the charge does not go through we will contact you.

Cancellation Policy: for month to month memberships you must cancel your membership 14 days in advance of your next withdrawal or your card will be charged. For yearly memberships there is no refund.

Must be 14 years old for a membership. For children younger than 14 please speak with Dale (937-336-4999) or Carrie (937-336-6563). Members under 16 must be accompanied by an adult.

I HEREBY ASSUME ALL THE RISKS OF PARTICIPATING IN ANY AND ALL ACTIVITIES ASSOCIATED WITH OUTER LIMITS GYM.

I hereby understand and acknowledge that the training, programs, facilities, events, activities and equipment offered for my use under or not under my membership contract may expose me to many risks, including but not limited to, accidents, injury, illness and even death. I assume all risks of injury(s) associated with participating in any and all manners including but not limited to falls, contact with other participants, coming into, being at or leaving the facility.

This document of waiver of liability and it’s part, or entirety, will not expire and will serve as the permanent agreement from this date forward.

I, after having read this waiver and understanding it in whole, knowing these facts, and in consideration of acceptance of my participation and any interaction with Outer Limits Gym, I agree for myself and anyone entitled to act on my behalf to indemnify, hold harmless, wave and release and promise not to sue Outer Limits Gym including but not limited to its officers, agents, ownership, investors, employees, organizers, appointed agents, insurers, landlord, representatives, and successors from any and all responsibilities, liabilities, demands, or claims of any kind arising out of my participation in Outer Limits Gym be it training, programs or events of any kind. I agree and understand that there are and will be no agreement outside of this agreement unless in writing.

I authorize Outer Limits Gym LLC to initiate a recurring charge to my credit card or checking account indicated above for the total amount due each month. I understand that I may cancel my recurring charge upon written notice to Outer Limits Gym LLC allowing a 14 day notice. I understand that should my card be declined or my check returned that I am still responsible for balances owed per my agreement with Outer Limits Gym LLC.
